Integrated care systems deliver joined up health and care services through a partnership of local organisations, coming together with the goal of planning and improving the health and wellbeing needs of the people who live and work in their local NHS ICS area.
In short, joined up care provision with partnership between statutory NHS organisations, local authorities, local councils, community and voluntary organisations working together in one integrated way to support the health of the community in which they are based.
